Job Description:

An exciting opportunity has arisen to join a dynamic and developing team within the Liverpool City Council; the Special Educational Needs (SEN) Assessment and Pupil Support Service. To continue Liverpool’s commitment of ensuring that children and young people with special educational needs and/or disabilities (SEND) have a positive educational experience, we are looking for 8 SEND Casework Officers to work in support of pupils with Education, Health and Care (EHC) Plans. The positions are on a 12-month fixed term basis during which time you will support the team to make substantial improvements to the EHC plan review process and ensure all children and young people’s EHC plans are supporting them to achieve their desired outcomes.

Job Responsibilities:

  • To supervise and develop a team of staff including annual appraisals, PRD, and supervision.
  • To deliver an outcome focused service which supports step up/step down and discharge to assess.
  • To provide cover within the Hub in the absence of the Manager.
  • To carry out the administering of medication as and when required.
  • To assist the Manager with the preparation of staff rotas and annual leave.
  • To work with internal and external stakeholders to support timely discharge home.
  • To work in partnership with, or as part of an integrated team of allied Health Professionals. (AHP)
  • To oversee the implementation of care plans and discharge planning.
  • Act as a Person Cantered Planner to support services Users and Carers in the production of person-centred support plans.
  • Establish strong links with other agencies and professionals in order to develop future services and alternative natural resources/links.
  • To participate in multidisciplinary team meetings and decision making.
  • To assist Assessors in commissioning the most appropriate service in line with desired outcomes Senior Care Assistant
  • To take a proactive approach in assisting the development of further service options in the wider neighbourhood, including partnership working with Health.
  • Act as a Trusted Assessor providing equipment / minor adaptations as agreed by Resource Manager.

Qualification & Experience:

  • Qualification in medication administration and completing appropriate MAR charts
  • Experience of admission and discharge planning
  • Experience of supervising staff/experience of supervising in a social care provision
  • Experience of completing support & risk management plans
